چکیده

Objective 1. To derive and validate an accurate clinical prediction model (“home score”) to estimate a patient’s risk of group A streptococcal (GAS) pharyngitis before a health care visit based only on history and real-time local biosurveillance, and to compare its accuracy to traditional clinical prediction models composed of history and physical exam features. 2. To examine the impact of a home score on patient and public health outcomes.

Improved diagnostic accuracy of group A streptococcal pharyngitis with use of real-time biosurveillance.

BACKGROUND Clinical prediction rules do not incorporate real-time incidence data to adjust estimates of disease risk in symptomatic patients. OBJECTIVE To measure the value of integrating local incidence data into a clinical decision rule for diagnosing group A streptococcal (GAS) pharyngitis in patients aged 15 years or older. DESIGN Retrospective analysis of clinical and biosurveillance p...

Effectively Managed by Remote Control? I this issue, Fine and colleagues (1) propose a solution to managing the ever-present threat of group A streptococcal (GAS) pharyngitis. Adult streptococcal pharyngitis continues to worry and perplex clinicians and public health authorities. Which patients require evaluation, and which require treatment? Answering these questions would not only improve car...

Accuracy of rapid strep testing in patients who have had recent streptococcal pharyngitis.

BACKGROUND Some clinicians have questioned the accuracy of rapid diagnosis of group A streptococcal pharyngitis by commercial immunochemical antigen test kits in the setting of recent streptococcal pharyngitis, believing that the false-positive rate was increased because of presumed antigen persistence. METHODS We studied 443 patients--211 cases--who had clinical pharyngitis diagnosed as grou...
